چکیده

Department of Electrophysiology, Hamburg, Germany Catheter ablation of atrial fibrillation (AF), particularly longer lasting AF, often requires extensive ablation within the atria and their annexed structures. Immediate procedural success is commonly defined as termination of AF during ablation. While this end point is associated with fewer AF recurrences and maintained sinus rhythm, recurrent atrial tachycardias are common and present a fresh challenge for the electrophysiologist. These atrial tachycardias are often more symptomatic than the initial AF, but are invariably more suited to mapping and ablation with an excellent prospect of maintained sinus rhythm.

"Cristal tachycardias": origin of right atrial tachycardias from the crista terminalis identified by intracardiac echocardiography.

OBJECTIVES We sought to use intracardiac echocardiography (ICE) to identify the anatomic origin of focal right atrial tachycardias and to define their relation with the crista terminalis (CT). BACKGROUND Previous studies using ICE during mapping of atrial flutter and inappropriate sinus tachycardia have demonstrated an important relation between endocardial anatomy and electrophysiologic even...

Right atrial tachycardias related to regions of low-voltage myocardium in patients without prior cardiac surgery: catheter ablation and follow-up results.

AIMS There are only few descriptions of patients without prior cardiac surgery in whom a large low-voltage zone (LVZ) or scar is the electrophysiological substrate for various atrial tachycardias. We describe the electrophysiological and electroanatomic characteristics of unusual macroreentrant atrial tachycardias (MRATs) in seven patients with spontaneous right atrial (RA) scarring and present...

Proarrhythmic aspects of atrial fibrillation surgery: mechanisms of postoperative macroreentrant tachycardias.

BACKGROUND Surgery for the treatment of atrial fibrillation may be associated with early or late postoperative atrial arrhythmias. In many cases, the arrhythmias that occur early in the postoperative course may be related to pericardial inflammation or increased catecholamine levels and tend to resolve without further therapy. In contrast, late postoperative arrhythmias often are persistent, hi...

Use of non-contact mapping in the treatment of right atrial tachycardias in patients with and without congenital heart disease.

AIMS Right atrial (RA) tachycardias may involve several mechanisms other than typical isthmus-dependent flutters, particularly in patients with congenital heart disease (CHD) or structural heart disease. We aimed at investigating the clinical utility of non-contact mapping in the diagnosis and ablation of these complex arrhythmias. METHODS AND RESULTS Non-contact mapping was used to treat RA ...

Dual-loop intra-atrial reentry in humans.

BACKGROUND Dual-loop atrial reentrant tachycardias have not been clinically described. METHODS AND RESULTS Five patients (3 men, 2 women; mean age, 48+/-16 years) were studied 24+/-15 years after surgical closure of an ostium secundum atrial septal defect for drug-resistant atrial tachycardia. Complete tachycardia mapping was performed in the right atrium with multipolar catheters and a 3-dim...
